Initiation Kit Return Instructions

• Insert both Vacutainer tubes back into protective cardboard holder.
• Place protective cardboard holder and swab tube in Lab Guard specimen bag. Do not remove absorbent pad.
• Place Lab Guard specimen bag into return mailer bag and seal closed.
• Drop your return mailer bag off at a UPS Store and ask for a receipt with a tracking number for proof of shipment and tracking purposes. This must be done THE SAME DAY sample was collected.
